You can get the mnemosynes from different levels of undeads, mu-miyah and skeletons. The reward for the unlocked ones are slightly better. In order to unlock them you'll need to get the hearts from Wood Golems, Copper Golems and Obsidian Golems. There's a check for lockpick skill while carving the hearts. You can turn them in for healing kits, dispell gems and arrow heads at the three NPCs in Tufa. They're located at 13.3S 5.1E.

You can also turn the mnemosynes in for experience points. Head over to Samsur and run to the portal located at 2.6S 16.4E. No level restrictions for handing in the tiny mnemosynes, level 15+ for small mnemosynes and level 30+ for the large ones.
